An Ontario Health Team is a new service delivery model designed to integrate care delivery and funding, which will enable patients, families, communities, providers, and system leaders to better work together, innovate, and build on what is best in Ontario’s health care system. The goal is to provide better, more connected care across the province.
The Huron Perth and Area Ontario Health Team was officially formed in 2019 as a team of local healthcare providers that partner and deliver high-quality, coordinated care for their patients and their communities. We want community members to experience a continuum of care through transitions in their care journey.
Population Health Management
We are currently engaged in various projects, encompassing mental health and addictions, infection prevention and control, heart failure care, accreditation, and digital health transformation. These initiatives collectively aim to enhance healthcare quality, streamline care pathways, and foster collaboration across various healthcare sectors. The focus is on integrating services, adopting standardized approaches, and leveraging digital technologies to improve patient and caregiver experiences and outcomes in Huron Perth & area.
