  • Huron Perth & Area Ontario Health Team (HPA-OHT) Heart Failure Working Group: Leading the initiative with innovative strategies and patient-centered care models.
  • Huron Perth Healthcare Alliance Medical Program Council: Offering invaluable medical expertise and support.
  • Office of Distributed Education, Schulich School of Medicine and Dentistry: Ensuring the latest research and best practices are incorporated into our community healthcare services.
  • Stratford Internal Medicine Associates: Providing specialist insights and frontline experience managing heart failure.

This initiative is underpinned by the generous financial backing of the Continuing Professional Development division of the Schulich School of Medicine and Dentistry, Western University, facilitating the development, dissemination, and evaluation of educational resources to improve community health outcomes.
