Mental health and addictions organizations are working together to provide access to a full continuum of care for members of our communities.  They are dedicated to mapping current services and the journey of care.

Infection Prevention and Control (IPAC)

The Huron Perth and Area Ontario Health Team (HPA-OHT) Infection Prevention and Control (IPAC) Program aims to improve the patient/client/resident and caregiver experience and reduce healthcare-associated infections (HAIs) through a standardized integrated approach to IPAC across all OHT partner organizations in Huron and Perth.

Heart Failure

Heart failure care for Huron, Perth & Area patients is provided through a collaborative model with three levels of care. As patients and caregivers move through the heart failure care pathways, they will encounter a team of health care providers working together with the shared interest of improving health and quality of life.

Accreditation

Our accreditation collaborative is dedicated to improving quality together. Organizations across six sectors are participating in a collaborative accreditation process with Accreditation Canada. The process is bringing together leaders, governors, staff, volunteers, patients, families, and caregivers to share expertise and knowledge of the organizations and the ability to align processes and policies.

Digital Health

Huron Perth & Area Ontario Health Team (HPA-OHT) is working to transform the digital health landscape in Huron Perth and Area. Digital health includes digital care programs and technologies to enhance the efficiency and experience of healthcare delivery and to make care more personalized and precise.

Stepped Care Inventory

The HPA-OHT Mental Health and Addictions Working Group collaborated on implementing a Stepped Care model for Huron Perth and Area. This Stepped Care Inventory provides information on programs and services offered in Huron Perth and Area and is intended to be used to support transition, referral and self-management for patients, caregivers, and providers seeking Mental Health and Addictions supports.
