We’re excited to present the LEAP™ Long-Term Care training course, a fully funded educational opportunity designed to enhance palliative care competencies across Ontario. Delivered in collaboration with Pallium Canada, this program is in line with the Model of Care for Adults Receiving Palliative Care in the Community and the Ontario Palliative Care Network’s Competency Framework.

Course Details

  • Date: April 30th & May 1st, 2024
  • Time: 4:00 PM to 7:00 PM ET
  • Format: Online (Virtual Workshop)

Course Content

LEAP Long-Term Care is an interprofessional course tailored for long-term care settings. It combines approximately 6-8 hours of prerequisite online modules with a 6-hour interactive, facilitated workshop. The course encompasses essential palliative care competencies and case studies, providing comprehensive training for healthcare professionals.

Accreditation

The course is accredited through:

  • College of Family Physicians of Canada: 26.5 Mainpro+® credits
  • Royal College of Physicians and Surgeons of Canada: up to 13.75 hours Upon successful completion, participants will receive a nationally recognized LEAP Certificate.

Registration Process

  1. Create or Login to your Pallium Central account.
  2. Enter Enrollment Key: OHWT-LTC-0430
  3. Access Course Materials: Navigate to ‘My Courses’, then ‘Course Work’.
  4. Complete Pre-Course Work: Ensure to finish the required pre-work before April 30th at 4 PM.

  • Palliative Care Comfort Scale
  • Palliative Care Knowledge Quiz
  • Online self-learning modules covering topics from Advance Care Planning to Suffering, Spiritual Care, and Maintaining Hope.
