Why it matters
A stronger, unified voice for primary care is coming soon to Huron & Perth. The PCN is a collaborative model that unites primary care clinicians and specialists across Huron Perth & Area. Its purpose is to drive system integration, ensure equitable resource allocation, and foster collaborative decision-making, all with a strong primary care focus at local, regional, and provincial levels. By building this foundation, we can deliver better, more connected care for patients and create a supportive network for providers that strengthens the entire health system. More information on the PCN will be shared in the coming months.
Dig Deeper
The Clinical Leadership Council (CLC) will serve as the executive team guiding the PCN.
We are currently recruiting for the CLC, which will include:
- 2 Family Physicians
- 2 Primary Care Nurse Practitioners (NP)
- 1 Specialist Physician
- 1 Executive Director
- 2 Flexible Clinical Representatives (any physician or NP)
- 3 HPA-OHT Implementation Committee Representatives
- Patient Advisors, as needed
