The HPA-OHT Palliative Care organizations came together over a year ago to apply to Ontario Health to obtain a clinical coach for the region, led by the OHT. The Palliative Care organizations and the OHT have continued to meet as a Community of Practice to advance resource material, share knowledge and now advance the integrated approach to connected services.
Danielle Hopf-Stasso, RN, BScN, joins us with nearly 20 years of nursing experience across oncology, long-term care, and education. She has:
- Led quality care programs in over 50 LTC homes
- Served as Director of Quality Resident Care at PeopleCare
- Taught at Conestoga College
- Delivered system-level projects in pressure ulcer prevention and behavioral supports
- Advanced primary-level palliative care with LEAP training, Desouza certifications, and a strong QI background
In her role as Clinical Coach, she will:
- Mentor and coach frontline providers across sectors
- Help identify improvement opportunities and support system change
- Use data to drive sustainable, equity-informed care models
- Champion person-centered, culturally responsive approaches
