The Huron Perth & Area Ontario Health Team (HPA OHT) is focusing on Mental Health and Addictions as a priority for population health management and system enhancements. These services are for members of our communities that are struggling with mental health and addictions and involve various sectors within the healthcare system to support their care.
The HPA OHT accesses expertise from local, regional, and provincial organizations to establish the full continuum of mental health and addictions care. We are working together to create seamless services to those accessing the mental health and addictions services with no wrong door to start the journey to care.
