Huron, Perth & Area patients with Heart Failure are a priority for population health management and system enhancements.   These patients are medically complex and involve various sectors within the healthcare system to support their care.

The Huron, Perth & Area Ontario Health Team (HPA-OHT) accesses expertise from local, regional and provincial organizations to establish the full continuum of heart failure care and, in April 2022 move towards a spoke-hub-node system model for care.
