The HPA-OHT Mental Health and Addictions Working Group collaborated on implementing a Stepped Care model for Huron Perth and Area. This Stepped Care Inventory provides information on programs and services offered in Huron Perth and Area and is intended to be used to support transition, referral and self-management for patients, caregivers, and providers seeking Mental Health and Addictions supports.
The Stepped Care model was developed in Canada by Dr. Peter Cornish and employs a person-centred approach that organizes evidence-based programming aligned with recovery principles (Mental Health Commission of Canada, 2021). The Stepped Care model improves equitable access to a variety of mental health resources and its framework enables the appropriate distribution of timely and accessible care, including the flexibility to meet people’s wide-ranging needs and preferences (Mental Health Commission of Canada, 2021). In doing so, the model aims to find the right balance between wellness promotion, illness prevention, treatment intensity, and risk management (Mental Health Commission of Canada, 2021).

This step involves intensive group programming and/or training sessions that are professionally led, and participating clients are initially referred by a clinician.

Flexible Intensive Individual Programming

This step involves more intensive one-on-one counselling sessions. Individual therapy is intense work during which clients are asked to set clear goals and work towards these with therapist guidance. Counsellors are encouraged to use time creatively and with some flexibility.

Chronic Care and Specislaist Consultation

This step involves outpatient psychiatric consultation for individuals that don’t show progress at Step 8 and/or specialized community based services for clients with chronic mental health conditions that require long-term or prolonged intensive treatment.

Individuals who would find this group helpful are ready to actively engage in goal-oriented group-based treatment and experience symptoms of generalized anxiety or panic; are experiencing symptoms of generalized anxiety or panic which may limit the individual’s daily functioning and participation in normal or usual activities of daily living; are prepared to complete weekly CBT based homework and are committed to the regular practice of skills.

This 10 week group is a closed evidence-based group, designed to enhance individuals understanding of the connection between thoughts, feelings and behaviours.
This short-term treatment group aims to strengthen skills and strategies for increased coping and more adaptive ways of thinking, feeling and behaving.
DBT SKILLS Group
Using the DBT Training Manual developed by Marsha Linehan, this program is a broad-based cognitive-behavioural treatment for individuals who struggle with both over control and under control of emotions and associated cognitive and behavioural patterns.
The purpose of this group is to educate clients on how to assess and identify thoughts, feelings and behaviours and to provide education and training in the areas of mindfulness, interpersonal skills, emotion regulation and distress tolerance.
This group is suited for individuals who have difficulty with exhibiting self-destructive or impulsive behaviours and need assistance managing negative core beliefs and emotions will benefit from this group.
This group lasts 2 hours in duration and consists of group instructions and dialogue as well homework as assigned.

Sexual Abuse Treatment Program
The Sexual Abuse Treatment Program (SATP) is dedicated to providing comprehensive crisis support, counselling, therapy, information and advocacy services to individuals who have been sexually assaulted. The program promotes a person-centered approach to service delivery and respect for the person’s self-determination.
The program coordinates with other community supports whenever possible to support our clients. The program also provides education and training regarding sexual assault to community agencies.

Transition Bed Program
The Transition Bed Program operates 24 hours a day, 7 days a week. Transitional Case Workers work with the client and community supports to create and implement a plan for enhanced stability to meet urgent needs in a safe, therapeutic environment.

∙ Provide an appropriate response level to situational crisis involving individuals who are experiencing mental health, addictions and/or criminal justice system involvement

The Huron Perth Helpline & Crisis Response Team is available to all residents of Huron and Perth Counties, who are experiencing a mental health crisis.
The Huron Perth Helpline & Crisis Response Team provides crisis assessments, brief crisis therapy, education about mental health and addiction services, and treatment referrals.
This service is offered 24 hours a day, 7 days a week through the helpline and in-person assessments.

Eating Disorders Dyad
A pilot program where a Mental Health Clinician and Registered Dieitian meet once or twice all together to support patients with disordered eating/eating disorder who are either awaiting treatment or exploring options for treatment.These meetings are not treatment, but are psycho-educational and involve exploring topics related to self-help resources and education about what happens when bodies do not get enough fuel. The RD and SW/RPs are participating in Eating Disorders Ontario support sessions for FHTs.

Mending Through Mindfulness: Mindfulness Based Cognitive Therapy
8-week program is for anyone experiencing moderate anxiety/depression. It is also highly recommended for people with a history of depression. MBCT helps reduce stress and anxiety and prevents relapses in those who suffer from depression. It combines the practice of mindfulness meditation with the practical skills of learning to interrupt thought patterns that lead to negative spirals in mood.

This short-term treatment group aims to strengthen skills and strategies for increased coping and more adaptive ways of thinking, feeling and behaving.
DBT SKILLS Group
Using the DBT Training Manual developed by Marsha Linehan, this program is a broad-based cognitive-behavioural treatment for individuals who struggle with both over control and under control of emotions and associated cognitive and behavioural patterns.
The purpose of this group is to educate clients on how to assess and identify thoughts, feelings and behaviours and to provide education and training in the areas of mindfulness, interpersonal skills, emotion regulation and distress tolerance.
This group is suited for individuals who have difficulty with exhibiting self-destructive or impulsive behaviours and need assistance managing negative core beliefs and emotions will benefit from this group.

Assertive Community Treatment Team
The Assertive Community Treatment Team (ACTT) provides intensive treatment, rehabilitation and support services for individuals with serious mental illness and complex needs who find it difficult to engage in other mental health services. The goal is to support individuals in their recovery and their desire to live in the community.
ACTT is multidisciplinary and includes social workers, nurses, a vocational specialist, psychiatrist, peer support worker, and recreational therapist. They provide psychiatric treatment, administer and monitor medications, help to access community services, and assist individuals in their activities of daily living. Support is provided as frequently as required to ensure individuals are supported 7 days a week.

Huron Perth Ontario Telemedicine Network Primary Care Nurse Practitioners
The Huron Perth Primary Care Nurse Practitioner Program consists of two programs.
The Primary Care Nurse Practitioner Program provides services to frail, complex elderly, and individuals with mental health and/or addictions concerns.
The Homeless Access to Care Program provides access to primary care to individuals experiencing homelessness.

Prevention & Early Intervention Program for Psychoses
The Huron Perth Prevention & Early Intervention Program for Psychoses (PEPP) provides assessment and treatment planning for individuals who are displaying their first episode of psychosis, such as hallucinations, delusions, or disorganized thought, and who have either had no treatment or have been treated for less than six months.
The program offers:

Community Treatment Order Program
A Community Treatment Order (CTO) is an order issued by a physician to support individuals with serious mental health disorders in receiving the care and treatment they need to remain in the community safely.
The Ontario Mental Health Act and the Health Care Consent Act govern the use of CTOs in Ontario. A CTO consists of a comprehensive Community Treatment Plan with specific expectations for the individual and their supports with regards to their psychiatric treatment.
The CTO Coordinator does not provide direct support or care.

Child & Adolescent Psychiatry
HPHA Department of Psychiatry provides outpatient psychiatric care via a collaborative care model.
Psychiatrists work collaboratively with individuals during an episode of care to establish diagnosis, provide treatment recommendations and plan discharge from psychiatric services with subsequent follow-up with the referring primary care provider. The duration of an episode of psychiatric care is determined by clinical need.

CHO Community Homes for Opportunity
CHO is a supportive housing program for people with serious mental illness. The program is intended to assist Tenants by providing appropriate housing and support services to achieve and maintain stability in a home that is safe and affordable. The Program offers opportunities for Tenants to enhance the quality of their daily living, their personal growth and development and improve life skills through participation in a variety of activities and programs.
