Huron, Perth & Area patients with heart failure are a priority for population health management and system enhancements. These patients are medically complex and involve various sectors within the healthcare system to support their care.
The Huron, Perth & Area Ontario Health Team (HPA-OHT) accesses expertise from local, regional and provincial organizations to establish the full continuum of heart failure care.
In April 2022, an expansion of spoke-hub-node system model for care. Numerous local and regional health care providers and organizations are engaged in patient care and co-design of this model of care.
