Unlike other portals, ConnectMyHealth consolidates health records from multiple participating hospitals across southwestern Ontario, giving you a comprehensive view of your medical information. This includes records from hospitals you’ve visited within our network.
Some of the features of ConnectMyHealth include:
- Notifications: Get email alerts when new test results or reports are available.
- Customizable Dashboard: Tailor your dashboard to focus on the information that matters most to you.
- Visit Timeline: Easily track past and upcoming hospital visits.
- Record Sharing: Share your records with friends and family.
- (NEW) Proxy Access: Parents and Legal Guardians of 16-17 year olds, and Legal Representatives of individuals aged 18+, can register to become a Proxy User. This means the Proxy User may access ConnectMyHealth in place of the Patient to view the Patient’s Data; the authorized Proxy is able to use ConnectMyHealth in the same way that the Patient would otherwise have been able to.
Health Information Available
With ConnectMyHealth, you can view a wide array of health information, such as:
- Radiology reports and images (if available)
- Cardiology reports and images (if available)
- Laboratory test results
- Clinical reports, such as hospital discharge summaries
- Medication records from hospital visits
- Documented allergies
- Pathology and genetics reports
- Details of past and future hospital visits (if available)

Parents and Legal Guardians of 16-17 year olds, and Legal Representatives of individuals aged 18+, can now register to become a Proxy User. A “Proxy” is a person who, under certain conditions, can be authorized to access ConnectMyHealth in the place of a Patient.
