Why it Matters

To support the promotion of ConnectMyHealth, a digital health tool that provides patients with a free, online, single access point to view their health records from participating hospitals in southwestern Ontario, the Huron Perth & Area Ontario Health Team (HPA-OHT) has developed a suite of ready-to-use promotional resources for organizations.

The materials serve as a toolkit to help raise awareness and drive registrations for ConnectMyHealth through social media, email, and in-person opportunities. By using these resources, your team can save time while ensuring consistent and effective messaging across the HPA-OHT network.

How to Customize the Email Template

Update Organization Information:

Replace [Your Organization’s Name] with your organization’s full name in both the subject line and greeting.

Personalize Recipient Details:

  • If sending this email individually, replace [Individual Name] with each person’s first name.
  • If using a mass email tool, use the tool’s merge tag or first-name field inserter (e.g., {{FirstName}} or [[FNAME]], depending on the email platform) to automatically insert each person’s name.
  • Alternatively, if personalization is not possible or desired, consider using a generic greeting such as “Hello” or “Good Morning”

Review and Modify Content (if needed):

Although this template is designed for direct use, you may adjust wording or add specific organization details (e.g., contact information for support) to align with your organization’s communication style.

Ensure Clarity on Primary Care Records:

Note that ConnectMyHealth does not include records from Primary Care Providers (e.g., Family Physicians). Ensure this is clearly communicated, as noted in the template, to set accurate expectations for patients.

Add Hyperlinks:

Link the phrases “ConnectMyHealth Data Availability” and “ConnectMyHealth” to the appropriate URLs provided in the template for easy access.

Send to Targeted Patients:

Distribute this email to individuals who may benefit from accessing their hospital records through ConnectMyHealth. In Primary Care settings, include it as part of new patient onboarding information.

Please note that health records from your Primary Care Provider (e.g., Family Physician or Health Team) are not viewable in ConnectMyHealth.

Why ConnectMyHealth?

Unlike other portals, ConnectMyHealth consolidates health records from multiple participating hospitals across southwestern Ontario, giving you a comprehensive view of your medical information. This includes records from hospitals you’ve visited within our network.

Some of the features of ConnectMyHealth include:

  • Notifications: Get email alerts when new test results or reports are available.
  • Customizable Dashboard: Tailor your dashboard to focus on the information that matters most to you.
  • Visit Timeline: Easily track past and upcoming hospital visits.
  • Record Sharing: Share your records with friends and family.
  • (NEW) Proxy Access: Parents and Legal Guardians of 16-17 year olds, and Legal Representatives of individuals aged 18+, can register to become a Proxy User. This means the Proxy User may access ConnectMyHealth in place of the Patient to view the Patient’s Data; the authorized Proxy is able to use ConnectMyHealth in the same way that the Patient would otherwise have been able to.

Health Information Available

With ConnectMyHealth, you can view a wide array of health information, such as:

  • Radiology reports and images (if available)
  • Cardiology reports and images (if available)
  • Laboratory test results
  • Clinical reports, such as hospital discharge summaries
  • Medication records from hospital visits
  • Documented allergies
  • Pathology and genetics reports
  • Details of past and future hospital visits (if available)

Option 6 (about Proxy Access):

ConnectMyHealth provides online access to various types of health records from participating hospitals in Southwest Ontario, helping to empower your health journey and that of your loved one. It is secure, easy to use, and works on computers and mobile devices. There’s no app to download, and it’s completely free.

Parents and Legal Guardians of 16-17 year olds, and Legal Representatives of individuals aged 18+, can now register to become a Proxy User. A “Proxy” is a person who, under certain conditions, can be authorized to access ConnectMyHealth in the place of a Patient.
