LEGHO Leads the Way: A Smart Path from Hospital to Home

For leaders in member organizations | 3-min read

Why it matters

Programs like Let’s Go Home (LEGHO) are redefining what successful hospital discharge looks like — connecting health and community services earlier to keep people healthier at home and reduce pressure on acute care. It’s a scalable model with strong early results.

Next steps

→ Explore how your organization could adopt or adapt the LEGHO model.
→ Consider embedding a community Care Planner into your discharge planning team.
→ Share learnings with local hospital and community care partners to expand impact.

Dig deeper

LEGHO is a collaborative initiative between ONE CARE Home and Community Support Services and Stratford General Hospital (HPHA), launched in April 2024. The program embeds a community-based Care Planner (CP) in daily hospital discharge planning to streamline transitions to home.

How it works

  • A nurse from the community initiates care planning before the patient/client leaves the hospital.
  • Patients/clients get bundled community support services at no cost.
  • Caregivers receive coaching, resources, and preparation.

Why it’s effective

By integrating services at the point of discharge, LEGHO reduces friction and delays:

  • 18% fewer Alternate Level of Care (ALC) days in the intervention unit — helping free up hospital beds.
  • 0-day wait from referral to first CP contact (vs. 2.3 days in standard care).
  • 100% of patients/clients would recommend LEGHO.
  • Patients/clients received more tailored services due to deeper needs assessment.

Health system benefit

Discharge teams report better coordination, faster referrals, and a clearer understanding of available supports. LEGHO helps address social barriers that often delay discharge — easing pressure on hospitals and improving outcomes at home.

What Leaders Are Saying

“Time and time again in Huron Perth, I’ve seen that our commitment to care excellence and deep respect for every member of the healthcare team consistently drive positive outcomes.

With a strong history of collaboration and a shared willingness to innovate, we came together — hospitals, home and community care, patients, and families — to explore how we could reduce patient length of stay, support care closer to home, and ease pressure on our system.

What began as an idea, grounded in the Quintuple Aim, has become the LEGHO demonstration project — and its success is a direct result of the way we work together. Special thanks to the E1-500 inpatient team for stepping up to pilot this important work.”

Bottom line

LEGHO isn’t just about faster discharges — it’s about connecting to community-based care with better outcomes for people. As it expands in 2025, it offers a replicable model for delivering integrated, community-first care that keeps people in their home and well cared for.
