Huron Perth and Area Primary Care’s Commitment: 100% Attachment to Team-Based Care within Two Years

Why it matters

The Ontario government’s Primary Care Action Plan seeks to connect every resident to a primary care clinician or team by 2029. The Huron Perth & Area Ontario Health Team (HPA-OHT) has submitted one collaborative proposal with the goal to attach 13,615 currently unattached residents in Huron Perth and Area to team-based primary care, achieving 100% attachment within two years.
