Understanding Digital Connections & Patient Resources

In November 2024, the Huron Perth & Area Ontario Health Team (HPA-OHT) conducted a system-wide integration mapping to assess how different sectors share patient information and resources. This initiative was guided by the Ontario Health Teams: Digital Health Playbook and direct input from healthcare sector representatives.

The HPA-OHT Integration Mapping provides a clear picture of how organizations across our network connect digitally. By identifying both existing connections and gaps, we can enhance patient care and strengthen collaboration across sectors.

How Can My Organization Use This Mapping?

The system-level integration mapping highlights the complexity of digital connections across HPA-OHT, including:

  • The various digital tools and platforms used by member organizations.
  • The patient-facing resources available across our network.

The sector-level integration mapping takes a more detailed look at individual sectors, showing both extensive and limited digital integrations.

Organizations can use this mapping to:
✔ Increase awareness of available patient-facing resources.
✔ Understand the digital solutions used by other member organizations.
