Why it Matters
Across Huron Perth & Area, the Home First approach plays a critical role in supporting patients to recover and thrive in their homes whenever possible. Home First is a shared provincial philosophy that prioritizes person-centred care, timely transitions, and the right care, at the right time, by the right provider (and in the right place: home).
Optimizing access and movement through the healthcare system is a key strategic priority for our HPA-OHT, with a focus on seamless access across hospitals, primary care, and community services. The Home First approach helps bring this priority to life by aligning teams and practices so patients can return home safely and be supported there whenever possible.
Dig Deeper
One of our regional partners is gathering information from across the region to understand what’s working well in our region when it comes to implementing Home First initiatives. We are asking our member and partner leaders to share examples of strategies you have implemented, or are currently implementing, that support the Home First model of care in the following ways:
- Communication & Education – What internal communication and education strategies have you implemented to ensure staff understand the Home First approach and its rationale?
- Examples: communications plans, new staff onboarding, ongoing education (at huddles, within learning modules, etc.), physician engagement, etc.
- Tools & Resources – What educational tools and resources have been provided to staff (including physicians) and to clients, patients, and families to support understanding of the Home First approach?
- Examples: events, scripts, print materials, posters, webinars, web resources, etc.
- Guiding Documents – What guiding documents have you created to reflect the Home First approach?
- Examples: policies, standards, procedures, guidelines, care pathways, etc.
