As the Huron Perth & Area health system continues to evolve, advancing integration is key to delivering seamless, high-quality care of our communities. The Huron Perth & Area Ontario Health Team’s (HPA-OHT) All Members Meetings offer a valuable opportunity to pause and reflect, bringing together voices from across the region to strengthen coordination, improve access, and enhance patient outcomes.

On October 28, nearly 50 representatives from the HPA-OHT gathered for the quarterly All Members Meeting. The morning featured updates on strategic priorities, collaborative planning, and meaningful discussions about the future of integrated healthcare in our region.
The meeting opened with a land acknowledgement from Louise Lepp, Executive Director of Rotary Hospice Stratford Perth, who emphasized actionable steps toward reconciliation within her organization.
Strategic Progress Highlights
What we have achieved together:
- Unattached Care Clinics: Over 5,000 patients have accessed services, improving timely care in the community.
- Access & Attachment: Strategies are advancing to improve patient attachment and entry points into care and working toward a Health Home Model with an implementation plan drafted for the new year.
- Digital Coordination: Hypercare adoption and onboarding continues to run smoothly, and Health811 is being regularly updated to ensure accurate information for patients and providers.
- Population Health Analysis: Davis Pier’s analysis will guide future planning and resource allocation.
- Chronic Disease Pathways: Notable progress in developing a hypertension care pathway within the heart failure model.
- Mental Health & Addictions: Piloting single-session therapy training to expand access and responsiveness.
- Data-Driven Decision Making: More organizations are submitting data to Integrated Decision Support (IDS), strengthening system-wide insights.
- Accreditation: 11 organizations are actively participating in the Accreditation Collaborative, aligning standards and improving quality.
- Equity, Inclusion, Diversity & Anti-Racism (EIDA-R): Over 110 participants have enrolled in training, building cultural competency across the system.
- Workforce Development: Educator mapping is underway to support workforce planning.
- Primary Care Network: Plans are progressing to transition the Physician Advisory Council into a Primary Care Network, enhancing leadership and collaboration.
- Cross-Sector Collaboration: Increased participation in HPA-OHT working groups and communities of practice.
Survey Insights – How We Work Together
The HPA-OHT Secretariat shared results from the Health System Performance Network (HSPN) survey, which measures how effectively Ontario Health Team members collaboratively work together.
By highlighting strengths and identifying areas for improvement, the survey provides valuable insights that inform strategic planning and help strengthen collaboration across the health system.
Key findings for HPA-OHT include:
- Strengths: Shared Purpose (common goals and collective understanding) and Organizational Approach (balancing alignment and adaptability).
- Opportunities for Growth: Organizational Context (collaboration and learning environment), Communications and Management (effectiveness of member engagement), Leadership (building trust and fostering innovation).
Collective Action & Incident Management
Roundtable discussions focused on leveraging our collective strengths and reviewing the draft HPA-OHT Incident Management Plan, a key step toward coordinated awareness and response across the system.
