Why it matters
Huron Perth & Area Ontario Health Team (HPA-OHT) is equipping Family Health Teams (FHTs) and other member and partner organizations with tools to effectively promote ConnectMyHealth (CMH), a secure digital platform that lets patients access their health information. This initiative aims to boost patient engagement and healthcare accessibility across the community.

A newly branded email template, modeled after Niagara North Family Health Team’s successful outreach, is available for patient communication. The template provides clear instructions for CMH registration and highlights benefits like 24/7 access to health records. Access it here.
Promotional Materials
Printed and digital materials—posters, brochures, and digital assets—are available for display in common areas. These tools help initiate patient conversations about CMH and promote enrollment. Feedback and Support
To enhance support, HPA-OHT invites all organizations to complete a feedback survey on the effectiveness of these resources. Additionally, the ConnectMyHealth Program Office is available for:

- Patient Engagement Support: Assistance in building community connections to boost CMH participation.
