The Primary Care Physicians Enabling Integrated Care program, initiated by the Ontario College of Family Physicians (OCFP), is critical in bolstering our joint understanding and implementation of integrated care.
About the Program
This innovative program is tailored to support Mainpro+ certification for integrated care teams and OHTs. Participating in this program demonstrates your commitment to integrated care through OHT working groups, advisory councils, and other initiatives. You are critical in advancing our mission, exemplifying unwavering dedication to quality care and ongoing education within our broad network of family physicians.
Engage with the OCFP’s Certified Program: This program component is designed to facilitate your initial foray into integrated care. Participate in the OCFP’s certified program and earn Mainpro+ credits while facilitating initial discussions regarding developing integrated care teams. The OCFP created this program in partnership with the Association of Family Health Teams (AFHTO), the Ontario Medical Association (OMA), and the Section on General Family Practice (SGFP).
