About Unattached Care Clinics

The Unattached Care Clinics (UCCs) in Huron Perth & Area are a collaboration among the Family Health Teams of the Huron Perth & Area Ontario Health Team (HPA-OHT). These clinics aim to improve healthcare access for unattached patients—those without a family doctor or nurse practitioner—by offering comprehensive primary care services. UCCs are strategically located in Goderich, Listowel, Stratford, and Exeter to serve diverse communities efficiently.

Patients seen at UCCs can receive care for acute illnesses, chronic condition management, or preventive healthcare. Clinics operate independently of Family Health Teams, ensuring immediate care while maintaining the flexibility for unattached patients to transition into long-term healthcare relationships elsewhere when needed.

The UCC initiative supports emergency departments by redirecting non-emergency cases to accessible primary care services, alleviating pressure on hospitals while promoting health equity in Huron Perth & Area.
