Why it matters
The Unattached Care Clinic (UCC) is a new program for residents of Huron Perth and Area, providing timely access to acute, chronic, and preventive care for individuals without a family doctor or nurse practitioner. Developed collaboratively by local Family Health Teams and supported by the Huron Perth and Area Ontario Health Team (HPA-OHT), the UCC is helping to bridge the gap in primary healthcare in an effort to ensure that no one in our community is left without essential services.

- Refer unattached individuals in your community to the UCC by completing and submitting the referral form on their behalf.
- Direct unattached individuals in your community to the UCC webpage or have them call the nearest clinic for appointments.
- Download and share a promotional poster to help spread the word or contact derek.barnes@hpaoht.ca for printed postcards to share with your patients/clients/residents.
