On October 22, 67 participants—including members, collaborating partners, patients, family, caregivers, indigenous advocates, nurse practitioners and physicians—convened to reflect on recent achievements and identify strategic shifts for the future. Focusing on four core strategic directions, participants outlined specific, actionable priorities:
1. Optimize Access and Integrated Care
Expand Care Entry Points: Simplify system navigation with a “no wrong door” approach and expand beyond traditional access points like primary care and ED.
Streamline Data and Resources: Integrate data for seamless information sharing, eliminate duplication, and coordinate technology use across the network.
2. Reimagine Community-Based Care
Shift Focus to Prevention: Transition from reactive care to proactive health-promotion strategies, prioritizing community-driven care and self-management.
Strengthen Caregiver Support: Build integrated models that emphasize home and community care to reduce pressure on primary and acute care facilities.
3. Revitalize the HPA-OHT Workforce
Drive Workforce Support and Equity: Focus on recruitment, retention, and sustainable funding models, including better support for international graduates and reducing workforce limitations.
Enhance Shared Leadership: Streamline administrative processes and promote collaboration to reduce burdens and support long-term workforce planning.
4. Advance the HPA-OHT Model
Build Unified, Regional Collaboration: Strengthen cooperation among sectors and eliminate competitive divides. Focus on shared resources, regional planning, and addressing funding disparities.

Outcomes from this meeting will guide the operational planning and be reviewed by the OHT’s leadership bodies for further action. In particular:
- Operational Alignment: The updated plan will be assessed by the Decision Support Working Group to incorporate meeting insights.
