Advancing Palliative Care: Building Capacity Through Clinical Coaching

Why it Matters

The Palliative Care Clinical Coach work is making meaningful progress in enhancing care for patients with complex needs. Over the last quarter, four Coaching and Assessment Tool (COAT) assessments have been successfully completed, two with Community Paramedicine teams and two with Primary Care providers. These assessments have provided valuable insights into current practices and identified opportunities for improvement.

Early identification of palliative patients is critical for ensuring timely, compassionate, and coordinated care, and findings from these assessments and the subsequent action plans equip clinicians with tools and knowledge to streamline processes and engage proactively with patients and families.

This work aligns with Ontario Health’s Adult Community Model of Care, which focuses on strengthening palliative care delivery in community settings through:

  • Clinical Coaching: Adding experienced Clinical Coaches to mentor and support providers in community organizations.
  • Education and Competency Development: Training aligned with the Ontario Palliative Care Competency Framework to build skills and confidence in primary-level palliative care.
  • Integrated Approach: Collaborating across sectors, including long-term care, family health teams, and community health centers, to ensure consistent, high-quality, person-centered care.
