The April 22, 2025 All-Members Meeting (AMM) reinforced the Huron Perth & Area Ontario Health Team’s (HPA-OHT) focus on transforming local health care delivery. With clear performance frameworks and strategic alignment, member organizations are moving forward with a shared vision for better access, integration, and community impact.

1. Strategic Progress & Operational Tools

  • Accreditation Collaborative: 11 member organizations, including HPA-OHT, are engaged. A new four-year cycle has begun, with the next on-site visit set for April 2028. Onboarding is open for new members.
  • Performance Measurement Framework: Developed in 2024 to support evidence-based planning. It’s tied to Integrated Care Pathways and the HPA-OHT Impact and Investment Framework. A baseline population dataset is expected in May 2025, with quarterly performance reviews aligned to strategic priorities.

2. 2025–2026 Operational Plan

Plan Finalized & Implemented: Includes measurable KPIs across four strategic outcome areas:

  1. Optimize Access and Integrated Care
    Metrics:
    • Number of unique patients accessing care at the Unattached Care Clinic
    • Use of eReferral across and between sectors
    • Percentage of sectors using Hypercare and number of active users
  1. Reimagine Community-Based Care
    Metrics:
    • Increase in preventative cancer screenings
    • Mobile Crisis Rapid Response Team (MCRT) diversion rates
    • Emergency department visits per 100 heart failure patients
    • Number of organizations/sectors onboarded to the In-Depth System (IDS)
    • Percentage of member organizations participating in HPA-OHT-led continuous improvement (e.g., accreditation)
  1. Revitalize the Workforce
    Metrics:
    • Percentage increase in clinical trainees across the OHT
    • Number of participants in Equity, Inclusion, Diversity, Anti-Racism (EIDA-R) training
  1. Advance the HPA-OHT Model
    Metrics:
    • Percentage of member organizations adopting joint HPA-OHT policies
    • Number of clinicians participating in the Clinical Leadership Council
    • Number of member organizations achieving system-wide accreditation

3. Sector-Wide Digital Integration Mapping

  • New Resource: Digital integration maps were shared to help visualize electronic medical records (EMRs), hospital information systems (HIS), and eReferral workflows across the region. These tools aim to improve system-wide collaboration and decision-making.

4. What’s Ahead

  • Member Feedback Survey Coming Soon: Will focus on equity, internal communications, and opportunities for continuous improvement.
  • Upcoming Integrated Care Projects:
    • Improving access to primary care and attachment to providers
    • Addressing social determinants of health and care navigation
    • Future project in chronic disease management (to be defined)
