Alzheimer Society Huron Perth (ASHP) is a non-profit charitable organization that actively supports families and individuals affected by Alzheimer’s disease and other dementias. They advocate for and provides support services, education, and funding for research for those affected by Alzheimer’s disease and other dementias.
Cheshire provide attendant services to individuals with permanent significant physical disabilities in London and in six surrounding counties. There are separate programs within two program types: Attendant Outreach and Community Assisted Living.
Cheshire is a member of the Leonard Cheshire Global Alliance’s family of services and globally there are over 200 Cheshire Homes and services in 50 different countries. The Alliance members run autonomous projects, services and campaigns and work to support disabled people and their right to participate fully in society.
Dale Brain Injury Services is an internationally recognized community based rehabilitation organization that provides exceptional services to individuals and families living with the effects of an acquired brain injury.
The objective of all of our services is to work with each client to maximize their potential for independence, reintegration into their community, vocational and avocational success, and positive relationships. Under the supervision of a registered Clinical Psychologist every client’s service plan is outcome based. These plans are regularly evaluated and updated.
Epilepsy Southwestern Ontario is a registered not-for-profit, charitable agency dedicated to enhancing the lives of people who are affected by epilepsy through support services, education, advocacy, and community awareness.
Epilepsy Southwestern Ontario provides a range of services to those affected by epilepsy in Erie-St. Clair and Southwest LHIN region (Huron, Perth, Grey, Bruce, Oxford, Elgin, Thames Valley, Windsor, Essex, Sarnia, Lambton and Chatham-Kent counties).
About 14,000 people in this area have been diagnosed with epilepsy. The effect of an epilepsy diagnosis goes beyond the individual; it impacts the entire family.

Hospice is a concept of care with a focus on caring, not curing, living not dying and helping to provide quality of life until the end. Hospice is all about HOPE.
Almost 30 years Huron Hospice has provided compassionate care, emotional support and practical assistance to individuals and families who are facing a life-limiting illness, extending through to the bereavement process. Care can be provided in a home, a hospital, a long-term care setting or our hospice residence. Our Volunteers offer relief to enable primary caregivers to feel comfortable to take time away or rest. Support is also provided to caregivers and families who are grieving the loss of their loved one.
Huron Hospice provides services and programs to residents of Huron County at no cost to the participants. However, we rely on donations to fully fund our program, which is partially funded by the Ministry of Health and Long Term Care.

ONE CARE Home & Community Support Services is a community based, charitable, not-for-profit health organisation providing supportive care to families in Huron and Perth Counties and surrounding area. The programs we offer help older people and people with health challenges to live at home within a network of support and in a caring community.
The Rotary Hospice Stratford Perth provides a peaceful, comfortable home with 24-hour specialized palliative care for those at end-of-life as well as support for those in their circle. It currently operates 8 beds in its hospice home, with support provided in areas such as clinical care, Care & Bereavement social workers, volunteers and legacy work. Their Visiting Hospice program supports a team of highly trained, compassionate, and knowledgeable volunteers who make weekly visits to client’s homes. The focus of the visit is to provide emotional and social support to individuals living with a life limiting illness and care for their caregivers. The one-to-one relationship allows time to build trust and rapport with each other, creating space for the volunteer to become a valued member in their circle of care.

With an annual operating budget of $148 million, the corporation has over 1,100 staff represented by three unions and a non-union group, over 500 volunteers and patient partners, and a growing number of medical staff.

Canadian Mental Health Association (CMHA), Huron Perth Addiction and Mental Health Services is a local mental health and addictions agency which provides a variety of services, including addiction counselling for youth and adults, mental health case management, peer support, court support, counselling for problem gambling/ behavioural change, supportive housing, community withdrawal management and more. Our goal is to provide quality services and easier access for those impacted by mental health and/or addiction issues in the communities across Huron and Perth.
The Huron-Perth Centre for Children and Youth is an accredited community-based children’s mental health centre. We provide assessment and treatment services for a wide range of mental health concerns for children and youth. We have been in operation since 1977 and have developed strong, positive working relationships with our valued community partners.
In addition to our child and youth mental health services we have a number of other youth related services and two services for adults specific to violence in intimate relationships.
We have developed several services that are purchased from community partners to meet a specific need. We believe that positive relationships are the foundation for healing and for realizing the potential in all persons.

Clinton FHT is a medical care team focused on family and community health. Their team includes family doctors, nurses, nurse practitioners, registered dietitian, social worker, mental health worker, psychologist, pharmacist and other health care providers and agencies in the community.
Clinton FHT has a Memory Clinic provided by their pharmacist, social worker and nurse practitioner. It serves to evaluate memory/attention of identified patients for the purpose of diagnoses, treatment, and planning.
Clinton FHT joined the HPA-OHT as it aligns with their mission to partner and collaborate with the patient, caregivers and community services to improve health outcomes.

The Grand Bend Area Community Health Centre (GBACHC) is a non-profit, community-governed organization that provides comprehensive primary health care and interprofessional health services.

The Listowel-Wingham and Area Family Health Team (LWA FHT) is a not-for-profit organization, funded by the Ministry of Health to provide primary care services to the residents of North Perth and North Huron and area through various healthcare services and programs. The LWA FHT amalgamated on April 1, 2021. Prior to this amalgamation, the two predecessor Family Health Teams (North Perth FHT and North Huron FHT) worked very closely together since their inceptions in approximately 2008.
We employ approximately 30 people, including Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Social Workers and Mental Health Counsellors, Pharmacists and Dietitians.
The FHT partners with other local health agencies in carrying out many of our programs. These partnerships are highly valued, as they improve and enhance the quality of the programs and services. We are members of the Huron-Perth and Area Ontario Health Team (HPA OHT), a collaborative of 60+ healthcare organizations in Huron and Perth whose vision is to create “a sustainable people-driven system that strives to provide a positive experience for all.”

Blue Water Rest Home (BWRH) was founded in 1965 as a charitable, not-for-profit Home for the Aged. Located in Zurich, in the southwest portion of Huron County, the Home provides high quality long-term care and support.
Situated just a few miles from the shores of Lake Huron and with views across the local fields and countryside, Blue Water Rest Home has become popular for seniors looking for a friendly care home in a rural setting.
Community Living North Perth is a not-for-profit registered charity. We champion and advocate for the inclusion and citizenship of people with developmental disabilities in North Perth.
Community Living North Perth began in 1958 as a group of concerned parents in need of a school for their children with developmental disabilities. It has evolved into an organization that is committed to providing unique, individualized support to people with a developmental disability in partnership with their families, friends and the larger community.
Today, our supports can range from 30 minutes a week helping to balance the cheque book to 24 hours a day helping with cooking, personal care, and anything else anyone needs to do in a day. We listen to what a person wants out of life and then help them formulate a plan to reach their goals and dreams.
Community Living North Perth is partially funded by social service, as well as by donations, fundraising, and community support.
Kingsway Lodge Nursing Home is a continuum of care that offers three levels of care, including independent living, supportive living, and assisted living. With these three levels, they are able to provide services that meet the needs of their residents as they transition through life. They are currently developing a 128 long term care bed home.
Kingsway Lodge joined the HPA-OHT to create a voice for long term care through the constant changes of Health Care in Ontario.

Huron Perth Public Health (HPPH) is one of 34 Ontario local public health units, covering the areas of Huron and Perth Counties. HPPH’s mission is to take action to protect and promote population health and prevent disease. HPPH has multiple public-facing teams in the areas of health protection, population health and community and family health.
HPPH provides a wide variety of services to its population. Foundational services include population health assessment, health equity, effective public health practice and emergency management. Clinical services include dental/oral health, sexual health/harm reduction, mother and young child clinics, health babies healthy children and vaccinations. Non-clinical services include inspections, safe food handling education, beach water monitoring, infectious disease response and Infection Prevention and Control (IPAC) audits. They manage and reduce health inequities through system level strategies. Their ongoing efforts to understand population health and the needs of communities address the social determinants of health.
HPPH joined the HPA-OHT to value relationships and commit to authentic engagement in their work in order to meet public health needs of communities. They endeavor to bring a population health-level lens to OHT collaborations, to highlight upstream strategies within OHT priorities, and advocate to support vulnerable community members.
