This page provides a high‑level overview of HPA‑OHT’s strategic planning process and will be updated as the work progresses.
Why it Matters
The Huron Perth & Area Ontario Health Team (HPA‑OHT) is developing a new Strategic Plan to set a clear, shared direction for the years ahead. The plan will define our collective mission, vision, and values, and establish measurable priorities that guide how we work together to advance our health system.
A refreshed strategic plan helps ensure alignment across the system, supports strong stewardship, and clarifies what HPA‑OHT is collectively accountable for delivering in support of better health outcomes for our communities.

HPA‑OHT’s current 2023–2026 Strategic Plan was extended by one year to allow time for a thoughtful and inclusive planning process. This work will result in a new Strategic Plan, Mission, Vision, and Values by the end of June 2026 and is being supported by Maria Sánchez Keane (Centre for Organizational Effectiveness).
The planning approach is designed to move from understanding system context and priorities, to validating and refining strategic directions, and ultimately to developing a clear, actionable plan that supports implementation across the system.
A summary of the work completed that supported achievement of the 2023 – 2026 strategic plan objectives is available to support continuity and reflection as future priorities are shaped.
What’s been done (as of April 20, 2026):
Several foundational steps have already been completed to support a strong and well‑governed planning process, including:
- Completion of a formal Request for Proposals (RFP) to support the development of a new Strategic Plan
- Selection of an external consultant, led jointly by HPA-OHT’s Implementation Committee and Planning & Priority Setting Committee
- A decision by system leadership to extend the current Strategic Plan by one year to allow time for meaningful engagement and plan development
These steps ensure the strategic planning process is transparent and grounded in system leadership.
What’s coming up:
Phase 1: Discovery (April 2026)
- Deliverable: Discovery Report
- This phase focuses on understanding what’s working, where gaps exist, and what matters most moving forward across the system. This phase includes extensive consultations through reviewing documents, data, and resources, and engagement through meetings, surveys, and a newly launching community engagement platform known as Social Pinpoint.
Phase 2: Validation & Refinement (May 2026)
- Deliverable: Validated Discovery Report
- Early findings and themes through engagement will be reviewed and refined to confirm strategic directions.
Phase 3: Development & Review (June 2026)
- Deliverables: New Strategic Plan, Mission, Vision, and Values
- Draft plans will be reviewed with system leadership, member organizations, Boards, and clinical and patient partners, with a focus on implementation and shared accountability.

- Continue to follow updates and engage in opportunities as they are shared through HPA-OHT communications
- Support community consultation by helping share information and opportunities to participate as they are released
