Ontario Health Features Huron Perth & Area Heart Failure and the Home IV Lasix Program

Why it matters

Heart failure (HF) continues to be a concern for residents in Huron Perth and Area and is one of the leading causes of hospital admissions, creating challenges for patients, families and health systems alike. In Huron Perth & Area, our Ontario Health Team (OHT) is committed to innovative, patient-centered solutions that keep care closer to home. The Home Intravenous (IV) Lasix Program is one of the initiatives helping us achieve these commitments.

This program allows eligible heart failure patients to receive IV diuretic therapy in their own homes, supported by the Heart Failure Clinic, Community Paramedicine, home care nursing, and with physician oversight. By intervening early, we help patients manage fluid overload without the need for hospitalization.

In Huron Perth & Area, this program is helping to avoid unnecessary emergency department visits and inpatient stays, easing pressure on hospitals and improving system efficiency. It’s a prime example of how integrated care can deliver better outcomes for patients and providers alike.

On December 4th, Community Paramedicine leaders Lori Vermeerch, Superintendent of Professional Standards and Community Programs for Huron County Emergency Medical Services (EMS) and Travis Coyle, Commander of Mobile Integrated Healthcare (MIH) for Perth County, proudly presented the success of this program at a Provincial Community of Practice meeting. The session was well attended, with strong engagement across the province, reflecting growing interest in replicating this model across Ontario.

Our region is leading the way in value-based care, ensuring patients receive timely, effective treatment while reducing avoidable hospitalizations. The Home IV Lasix Program demonstrates how collaboration and innovation can transform heart failure management locally.
