LEGHO is a collaborative initiative between ONE CARE Home and Community Support Services and Stratford General Hospital (HPHA), launched in April 2024. The program embeds a community-based Care Planner (CP) in daily hospital discharge planning to streamline transitions to home.
How it works
- A nurse from the community initiates care planning before the patient/client leaves the hospital.
- Patients/clients get bundled community support services at no cost.
- Caregivers receive coaching, resources, and preparation.
Why it’s effective
By integrating services at the point of discharge, LEGHO reduces friction and delays:
- 18% fewer Alternate Level of Care (ALC) days in the intervention unit — helping free up hospital beds.
- 0-day wait from referral to first CP contact (vs. 2.3 days in standard care).
- 100% of patients/clients would recommend LEGHO.
- Patients/clients received more tailored services due to deeper needs assessment.
