ONE CARE Home & Community Support Services plays a vital role in supporting older adults and individuals with health challenges in Huron and Perth counties. Among their key initiatives, ONE CARE has been instrumental in Let’s Go Home (LEGHO), an HPA-OHT-supported program designed to enhance hospital discharge planning. By integrating care planning with Stratford General Hospital’s medical unit, LEGHO ensures smoother transitions from hospital to home, helping individuals recover with the right support in place.
ONE CARE also played a key role in the HPA-OHT Accreditation Collaborative, achieving Exemplary Standing in Ontario’s first joint accreditation process with 9 other HPA-OHT member organizations. This recognition underscores their commitment to high-quality, integrated care and highlights the strength of collaboration across the local health system.
