The Four-Year Cycle: A Closer Look

Accreditation Canada’s continuous improvement model aligns perfectly with HPA-OHT member organizations’ missions, emphasizing:

  • People-centered care
  • A healthy workforce
  • Adaptability to change

Our new four-year cycle transforms accreditation from a periodic checkpoint into a daily practice:

Years 1 & 2: Foundation Building

  • Receive a customized assessment manual
  • Utilize tools like the HSO Global Workforce Survey
  • Create a Quality Improvement Action Plan
  • Conduct self-assessments
  • Align policies across the collaborative

Year 3: Self-Directed Assessment

  • Complete a comprehensive self-directed attestation
  • Gather data to inform third-party surveyors

Year 4: External Validation

  • Undergo a short-notice, on-site assessment
  • Third-party surveyors audit a sample of attestable criteria
  • Ensure sustained excellence in core standards

Benefits of Our Phased Approach

  1. Embeds quality and safety practices into daily operations
  2. Promotes a culture of continuous improvement
  3. Breaks the process into manageable steps
  4. Ensures continuous improvement is at the forefront of our leadership
