eReferral allows clinicians to send a referral electronically rather than by fax. eReferral provides standardized referral forms that are sent seamlessly and quickly to specialists and programs. You and your clinician can receive real-time updates about the status of your referral.
Online Appointment Booking allows you to request or directly schedule appointments with your primary care clinician using your desktop or mobile device. Online Appointment Booking improves the booking experience for residents, and enhances clinic efficiencies.
Cybersecurity training is providing the knowledge and tools to clinicians and staff across Huron Perth and Area to recognize and manage a breach if one were to occur. Cybersecurity training is comprehensive and ensures that healthcare teams are aware of best practices and privacy obligations to protect health information.
Hypercare is an application that provides secure, real-time instant messaging among members of a healthcare team, region or organization. Hypercare is PHIPA-compliant and facilitates secure communication between health care providers. Hypercare will improve real-time communication and integrated care delivery among the healthcare system in Huron Perth and area.
The ConnectMyHealth Patient Portal is a digital tool that will provide you with a secure, online platform to view some of your health records from many healthcare facilities in Ontario. This portal may support your visits across the health sector providing easy access to your records to share with your healthcare teams.

Evidence2Practice is working with clinicians, topic experts, and other system partners to develop a suite of free evidence-based tools that will integrate seamlessly into frontline systems, enabling acute and primary care clinicians to access the information they need easily and quickly at the point of care. The tools will also streamline the patient journey by including connections to community resources and patient self-management resources.
